چکیده
The management of sickle-cell crisis in a pregnant patient by exchange transfusion is described, the procedure leading to immediate and dramatic improvement in the condition. Partial exchange transfusion in three other patients with sickle-cell anaemia, judged by episodes of crisis in previous pregnancies to be at particular risk, is also reported and the value of this method of management discussed.

Intracardiac Thrombosis in Sickle Cell Disease
In patients with sickle cell disease, thrombotic microangiopathy is a rare complication. Also in sickle cell disease, intracardiac thrombus formation without structural heart diseases or atrial arrhythmias is a rare phenomenon. We herein describe a 22-year-old woman, who was a known case of sickle cell-βthalassemia, had a history of recent missed abortion, and was admitted with a vaso-occlusive...
متن کاملA modified transfusion program for prevention of stroke in sickle cell disease.
Regular red blood cell transfusions reduce the rate of recurrent cerebral infarction in sickle cell disease but lead to accumulation of excessive iron. We studied the effect on the prevention of recurrent stroke and the volume of blood transfused of a modified transfusion program in which the pretransfusion percentage of hemoglobin S (HbS) was maintained at 50%, rather than the conventional 30%...
متن کاملPreoperative transfusion in patients with sickle cell disease to prevent perioperative complications: a systematic review and meta-analysis.
BACKGROUND Sickle cell disease (SCD) is associated with perioperative vascular (SCD-related) and non-vascular complications. To minimize perioperative complications during elective surgery, either exchange blood transfusion or simple blood transfusion can be used. We systematically reviewed the literature and meta-analyzed randomized and observational trials comparing exchange transfusion to si...
متن کاملRed cell exchange in sickle cell disease.
Red cell exchange transfusions remain an effective but possibly underutilized therapy in the acute and chronic treatment of sickle cell disease. In sickle cell disease, increased blood viscosity can cause complications when the hemoglobin exceeds 10 g/dL even if this is due to simple transfusion. Red cell exchange can provide needed oxygen carrying capacity while reducing the overall viscosity ...
متن کاملNephrotic syndrome, sickle-cell disease and pregnancy.
A young West Indian with sickle-cell disease and the nephrotic syndrome in the last trimester of pregnancy was treated after admission to hospital with folic acid and blood transfusion. Labour was induced by intravenous prostaglandin E2 but the fetus died. The nephrotic syndrome resolved but the proteinuria, the cause of which it is suggested is a true sickle-cell nephropathy, persisted.
